• The Transcript Request Form is intended for current and former students to request grades, attendance or immunization records.  Submit the completed form directly to the student's school, if the student is currently attending that school.  During summer break, the request may be submitted to: Jefferson School District, 1219 Whispering Wind Drive, Tracy, California 95377.
