Student Registration
The following information is required to enroll your student via Aeries Online Enrollment:
- Proof of Residency: A current PG&E or City of Tracy utility bill, and one of the following: a mortgage statement, a filed deed to property, or a current signed lease/rental agreement.
- Immunization Record: A current and up-to-date printout from your doctor's office.
- Proof of Birth
- Parent/Guardian ID: A valid California Driver's License or Identification Card.
- Required Forms:
- Authorization for the Exchange of Information Form
- Review of Student Records Form
- Authorization for Emergency Treatment & Transportation Form
Special Education and 504 Plans
If your child receives Special Education Services or is on a 504 Accommodation Plan, please submit the following to the school secretary upon registration:
- A copy of the current Individualized Education Program (IEP)
- A copy of the 504 Accommodation Plan
California Immunization Requirements (TK-8th Grade)
Parent(s) or Guardian(s) must provide proof of the following immunizations when registering to attend school:
- Polio: 4 doses. Note: One additional dose may be needed if the child was under 4 years of age at the time of the last immunization.
- DTaP: 5 doses. Note: One additional dose may be needed if the child was under 4 years of age at the time of the last immunization.
- Hepatitis B: 3 doses.
- MMR: 2 doses.
- Varicella (Chickenpox): 2 doses. Note: Your child must be fully vaccinated to attend school. If your child has had chickenpox or is not fully vaccinated, a physician must provide a medical waiver (PDF, opens in new tab).
- Tdap Booster: 1 dose (Required for 7th-grade enrollment only).
Important Legal and Enrollment Notices
Legal and Guardianship Documents: If there is any legal or guardianship information regarding your child, please bring copies of the applicable legal documents. These documents must be on file in the school office.
Enrollment Capacity: Your home school may be overcrowded. Even though we are registering your child today, there is a possibility that your child may need to attend another school within the Jefferson School District.
