Registration

Required Information

Required information needed to enroll in Aeries Online Enrollment:

  • A valid address within JSD school district boundaries
  • General information about your student
  • Name, address, and district of previous school (if applicable)
  • Parent or Guardian's home, work, and mobile number, home address and valid email
  • Name and phone numbers of emergency contacts
  • Local physician name and phone number

California Immunization Requirements for TK – 8th Grade Registration:

Proof of Current Immunizations

Parent(s) or Guardian(s) need to provide the following information to the local school when registering:

Parents must show proof of the following number of immunizations:

•             4 Polio
•             5 DTaP
•             3 Hepatitis B
•             2 MMR
•             2 Varicella**
•             Tdap booster for 7th grade Enrollment

*One additional Polio or DPT may be needed if the child was under 4 years of age at the time of the last immunization.

** Your child must be fully vaccinated to attend school. If your child has had chicken pox,  or is not fully vaccinated a physician must provide a medical waiver.

 If your child receives Special Education Services or a 504 Accommodation Plan:

  • Submit copy of Individualized Education Plan to school secretary
  • Submit copy of 504 Accommodation Plan to school secretary

If there is any legal/guardianship information about your child, please bring copies of applicable LEGAL DOCUMENTS. These documents must be on file in the school office.

Your home school may be overcrowded. Even though we are registering your child today, there may be a possibility of your child attending another school within the district.
